Multan made a rapid start as Sahibzada Farhan and Smith added 71 runs in just five overs.
Farhan scored a brisk 32 off 14 balls before falling to Alzarri Joseph.
Josh Philippe departed soon after for 18, leaving the Sultans at 96/2 in eight overs.
Smith then combined with Shan Masood to steady the innings with a 40-run partnership.
Smith top-scored with a fluent 53 off 35 deliveries, striking seven fours and a six, before being dismissed in the 14th over.
Masood remained unbeaten on 46 off 30 balls, guiding the side home with two fours and two sixes.
For Quetta, Hasan Nawaz was the most effective bowler, taking two wickets for 17 runs, while Joseph and captain Saud Shakeel claimed one wicket each.
Nawaz leads bowling effort
Earlier, Multan’s decision to field first proved effective as Quetta were restricted to 166/7 in their 20 overs.
The Gladiators lost early wickets, with Khawaja Nafay (12) and Shamyl Hussain (2) dismissed inside the first three overs.
A 55-run stand between Hasan Nawaz (20) and Saud Shakeel helped stabilise the innings before Arafat Minhas struck twice in quick succession, removing Hasan and Rilee Rossouw to leave Quetta at 72/4.
Shakeel anchored the innings with a composed 56 off 41 balls, including six fours and a six.
He added 47 runs with Bevon Jacobs for the fifth wicket before falling to Mohammad Nawaz in the 17th over.
Jacobs remained unbeaten on 49 off 31 deliveries, hitting four fours and three sixes, but lacked support at the other end.
Nawaz led the bowling attack with figures of 3 for 30 in four overs. Minhas took two wickets, while captain Ashton Turner chipped in with one.
