Karachi made a brisk start with Saad Baig and Jason Roy putting on 29 runs for the opening wicket.
Roy scored 16 off nine balls before Arafat Minhas provided the breakthrough.
Minhas struck again soon after, dismissing Salman Ali Agha for two, leaving Kings at 41-2.
Saad Baig fell for 21 off 15 deliveries, with Peter Siddle reducing the hosts to 52-3 inside the powerplay.
Reeza Hendricks and captain Moeen Ali then rebuilt the innings with a 67-run partnership.
Hendricks made 49 off 32 balls before falling just short of a half-century, while Moeen contributed 27.
Multan regained control through regular wickets. Ashton Turner dismissed Moeen, while Momin Qamar removed Khushdil Shah cheaply.
Arafat Minhas continued his impressive spell, claiming three wickets including Shahid Aziz for a first-ball duck, leaving Karachi struggling at 160-7.
Abbas Afridi kept the chase alive with a quick 34 off 16 balls, supported by Hasan Ali’s late hitting.
However, Siddle removed Abbas at a crucial stage. Two run-outs in the final overs ended Karachi’s resistance, with Hasan Ali dismissed for 23 off 10 balls.
Earlier, Multan Sultans posted 207-7 after being asked to bat. Their innings got off to a shaky start when Khushdil Shah dismissed Steve Smith for a duck.
Awais Zafar and Josh Philippe steadied the innings with a 66-run stand. Philippe scored 44 off 23 balls, while Zafar added 36.
Shan Masood then played a key role, making 46 off 25 deliveries to keep the momentum going.
Khushdil was the standout bowler for Karachi, taking 3-35, while Moeen Ali picked up two wickets to slow the scoring in the middle overs.
Multan accelerated in the final overs, led by Mohammad Imran’s explosive unbeaten 26 off just eight balls, which included two fours and two sixes.
His late assault lifted the total past the 200-mark, ultimately proving decisive.
